In this energetic position, you will assess patient needs, develop personalized treatment plans, and collaborate with a multidisciplinary team to deliver exceptional outpatient and home health care. Your expertise will help patients regain independence and achieve their rehabilitation goals through innovative physiotherapy techniques and compassionate care. Responsibilities Conduct comprehensive patient evaluations to determine functional limitations and develop tailored treatment plans. Implement evidence-based physiotherapy interventions, including manual therapy, trigger point therapy, and therapeutic exercises. Manage discharge planning processes to ensure seamless transitions from inpatient to outpatient or community settings. Document patient progress accurately in electronic health records (EHR) systems while adhering to clinical documentation standards. Collaborate closely with healthcare professionals such as other physicians. Educate patients and their families on treatment goals, home exercises, and community resources to support ongoing recovery. Skills Strong knowledge of physiology, anatomy, and medical terminology relevant to physiotherapy practice. Skilled in treatment plan development for various conditions including orthopedics, neurology, geriatrics, pediatrics, and post-surgical rehabilitation. Familiarity with clinical documentation standards and electronic health records (EHR/EMR systems). Excellent client communication skills to motivate patients and explain complex treatment concepts clearly. Ability to work collaboratively with healthcare teams across multiple disciplines in fast-paced environments.
