Work Environment Your work will primarily take place in patients' homes, where you will facilitate rehabilitation in a comfortable and familiar setting. You'll collaborate with a multidisciplinary team, including other healthcare providers, to create comprehensive care plans that transcend traditional office boundaries. We uphold a professional standard of excellence, ensuring that every interaction is guided by respect, empathy, and patient-centered care. Key Responsibilities Conduct thorough evaluations and assessments of patients to determine their functional capabilities and limitations. Develop, implement, and modify individualized treatment plans based on evaluated needs and ongoing progress. Provide direct therapy services, including exercises, manual therapy, and patient education to maximize recovery and functional mobility. Participate in consultations with other healthcare professionals regarding the patient's continuum of care, adjusting treatment plans as needed. Accurately document each treatment session, including progress notes and charges for services provided, adhering to applicable documentation laws and professional standards. Engage with the interdisciplinary treatment team, maintaining open lines of communication to discuss patient progress and coordinating care efforts. Supervise and mentor assigned support staff, fostering an environment of learning and professional development. Participate in community outreach and engagement efforts to promote health and wellness within the community. Attend staff meetings and continuing education seminars to enhance your professional skills and knowledge. Patient Population / Client Focus The ideal candidate will work with a diverse patient population, primarily focusing on individuals recovering from orthopedic surgeries, injuries, and chronic musculoskeletal conditions. You will treat patients of all ages, adapting your approach to meet their unique needs and preferences while considering their cultural backgrounds. Your efforts will directly impact the health outcomes of your patients, guiding them from recovery through full functional restoration. Qualifications A Bachelor's Degree in Physical Therapy from an accredited institution is required, with a Doctorate in Physical Therapy preferred. Current state licensure to practice as a Physical Therapist in New Jersey is mandatory. Exceptional verbal and written communication skills are essential for effective patient interaction and collaboration with the healthcare team. Demonstrated ability to assess, evaluate, and deliver effective treatment plans tailored to individual patient situations. Strong organizational skills and the ability to manage multiple priorities in a fast-paced environment. Preferred Experience While new graduates are encouraged to apply, a minimum of 1-2 years of experience in orthopedic physical therapy is preferable. Familiarity with home care settings and an understanding of community health resources will be advantageous. Compensation and Benefits We offer a competitive salary range of
USD 100,000
to 120,000 per year, commensurate with experience and qualifications.
