Job Description

Job Summary We are seeking a dedicated and compassionate Physical Therapist to join our dynamic healthcare team. In this vital role, you will assess, plan, and implement individualized treatment plans to improve patient mobility, reduce pain, and enhance overall quality of life. You will work closely with a multidisciplinary team to deliver exceptional patient care across various settings, including outpatient clinics, inpatient hospitals, home health environments, and community care programs. Your expertise in physiotherapy and rehabilitation will empower patients of all ages—ranging from pediatrics to geriatrics—to regain independence and achieve their health goals. Responsibilities Conduct comprehensive patient evaluations to determine functional limitations and develop tailored treatment plans. Implement evidence-based therapeutic interventions such as manual therapy, trigger point therapy, and exercise programs to promote recovery. Collaborate effectively with healthcare professionals including physicians, nurses, occupational therapists, and social workers to coordinate patient care. Manage clinical documentation standards by accurately recording treatment progress, discharge planning details, and patient outcomes within electronic health records (EHR) systems. Provide education and guidance to patients and their families on exercises, mobility techniques, and home care strategies to support ongoing rehabilitation. Assist in discharge planning by assessing patient needs for continued therapy or community-based services post-treatment. Stay current with physiotherapy practices by participating in ongoing professional development and adhering to clinical documentation standards. Skills Strong knowledge of physiology, anatomy, orthopedics, neurology, pediatrics, geriatrics, and related medical terminology. Proven experience practicing in a physical therapy role within outpatient clinics, inpatient hospital settings, or home health environments. Expertise in developing effective treatment plans that incorporate physiotherapy techniques tailored to individual patient needs. Familiarity with outpatient rehabilitation center operations and outpatient clinic experience. Ability to work with diverse patient populations including geriatric patients and pediatric clients. Proficiency in electronic health records (EHR) management and EMR systems for accurate documentation of patient care. Excellent client communication skills to foster trust and motivate patients throughout their rehabilitation journey. Experience with discharge planning, care plans formulation, acute care experience, inpatient experience, hospital experience, outpatient experience, home & community care experience, and outpatient rehabilitation center experience is highly desirable. Pay:
$45.00 - $55.00 per hour
