Job Description
Job Summary We are actively seeking a dedicated and highly skilled Licensed Physical Therapist to join our dynamic healthcare team. The full time position is ONLY 3 days a week!! (MWF only ) This immediate opening offers a flexible three-day workweek, providing an excellent opportunity for qualified professionals to deliver exceptional patient care while maintaining work-life balance. The ideal candidate will possess comprehensive experience in outpatient, inpatient, or home health settings, with a strong focus on patient rehabilitation, treatment plan development, and interdisciplinary collaboration. You will play a vital role in enhancing patient outcomes through evidence-based physiotherapy practices and personalized care strategies. Responsibilities Assess patients' physical conditions, including geriatrics, pediatrics, orthopedics, neurology, and other specialties, to develop tailored treatment plans. Implement effective therapy interventions such as manual therapy, trigger point therapy, and exercise programs to promote recovery and improve mobility. Manage discharge planning processes by coordinating with healthcare professionals and ensuring seamless transitions across care settings. Document clinical findings accurately in electronic health records (EHR) systems while adhering to clinical documentation standards. Collaborate closely with physicians, nurses, and other healthcare providers to optimize patient care strategies and treatment outcomes. Conduct patient education sessions to enhance understanding of physiotherapy techniques and promote self-management at home or in community settings. Engage in ongoing professional development to stay current with advances in physiotherapy practices, physiology knowledge, and medical terminology. Support the development of comprehensive care plans that align with patient goals and clinical best practices across outpatient clinics, hospitals, or home health environments. Skills Extensive experience practicing in a physical therapy role within outpatient clinics, inpatient hospitals, or home health settings. Proficiency in clinical documentation standards and electronic health records (EHR/EMR systems) management. Strong knowledge of anatomy, physiology, orthopedics, neurology, and geriatrics to inform treatment planning. Ability to communicate effectively with diverse patient populations and multidisciplinary teams. Familiarity with outpatient rehabilitation center operations and outpatient experience is highly desirable. Expertise in discharge planning, care plan development, and patient rehabilitation techniques. Working knowledge of medical terminology relevant to physiotherapy and healthcare documentation standards. Experience working with geriatric patients or pediatric populations is an advantage for delivering targeted care.
