Manage electronic health records (EHR) and electronic medical records (EMR) systems to document patient progress accurately and efficiently. Coordinate discharge planning and collaborate with multidisciplinary teams to ensure seamless patient care transitions across settings. Provide patient education on injury prevention, treatment exercises, and home care strategies to maximize recovery outcomes. Assist in outpatient rehabilitation programs by guiding patients through therapy sessions aimed at improving mobility and strength. Monitor patient progress regularly, adjusting treatment plans as necessary to meet evolving needs and goals. Requirements Valid licensure as a Physical Therapist in accordance with state or national licensing boards. Proven experience practicing in a physical therapy role within outpatient clinics, hospitals, or home health environments. Strong knowledge of physiology, anatomy, medical terminology, and clinical documentation standards. Experience working with diverse populations including geriatrics, pediatrics, neurology patients, and those requiring orthopedics or inpatient care. Familiarity with outpatient rehabilitation center operations and inpatient hospital workflows. Ability to communicate effectively with patients and collaborate seamlessly with healthcare professionals across disciplines. Proficiency in managing electronic health records (EHR) systems such as EMR platforms for accurate documentation of treatment plans and progress notes.
