Do you have experience working within aHome Health Agency or working in a professional healthcare facility? JFSA Caring at Home has an immediate opening for a PRN Physical Therapist to provide comprehensive, professional, and compassionate PT services to clients in their homes. The Physical Therapist assesses and treats clients according to the individualized plan of care; monitors and documents the client's condition, progress, and outcomes; and provides client and caregiver. Services are provided under a physician's order and in accordance with home health agency regulations, federal and state laws, and agency policies.
Duties & Responsibilities:
Provides PT assessments, diagnostic tests, and skilled treatments, and ongoing evaluations of clients who are receiving services under a medically approved Plan of Care. Participates in the development of the Plan of Care. Guides and instructs the client in prescribing therapeutic self-care and creative activities that are directed toward improving independence and physical and cognitive functioning. Prepares a report of each clients diagnostic evaluation and makes recommendations. Prepares clinical and progress notes. Instructs clients and their families in Activities of Daily Living. Maintains confidentiality in all aspects of the job. Attends care conferences as deemed necessary. Participates in in-service programs as requested. Performs other duties as assigned.
