The primary purpose of this position is to provide skilled physical therapy services to patients in their homes, including: Evaluating patient needs and developing individualized treatment plans. Delivering therapy interventions to improve mobility, function, and quality of life. Collaborating with physicians, nurses, and other members of the care team. Educating patients and families on safe exercises and techniques. Documenting care accurately in the electronic health record (EHR) system. Home health experience is preferred, but not required—we are happy to provide training and support to help you grow in this specialty. Strong communication skills, clinical expertise, and a commitment to patient-centered care are essential. EDUCATION
Licensed PT with current OH state license.
EXPERIENCE
We are looking for clinicians who are passionate about patient care and eager to make a meaningful impact. Home health experience is a plus, but not required—we are happy to share more about this rewarding field with candidates who are excited to learn. The ideal candidate will have strong communication skills, the ability to build trusting relationships with patients and families, and the professionalism to coordinate care with our team. Excellent organization and time management skills are also essential, as you'll have the autonomy to plan your schedule and balance multiple priorities throughout the day.
