What You'll DoAs a Home Health Physical Therapist, you will evaluate, develop, and implement individualized treatment plans that improve mobility, reduce pain, restore function, and help patients safely remain in their homes.

Responsibilities include:

Perform comprehensive physical therapy evaluations and establish individualized plans of care.

Develop and implement treatment programs to improve strength, balance, mobility, and functional independence.

Reassess patient progress and modify treatment plans to achieve the best outcomes.

Educate patients and caregivers on therapeutic exercises, safety techniques, and injury prevention.

Collaborate with physicians, nurses, and interdisciplinary team members to coordinate patient care.

Maintain accurate, timely documentation in accordance with agency policies and regulatory standards.

Follow infection prevention and patient safety protocols during every visit.

Promote exceptional patient care while representing Choice Health at Home with professionalism and compassion.

QualificationsGraduate of an accredited Physical Therapy programCurrent Physical Therapist (PT) license in the state of practiceCurrent CPR CertificationValid driver's license and reliable transportation with current auto insuranceMinimum one year of Home Health experience preferredHomeCare HomeBase experience preferredPhysical RequirementsAbility to stand, walk, bend, kneel, squat, climb, and lift throughout the workday.

Ability to lift, carry, push, or pull up to 100 pounds with appropriate assistance when necessary.
