What You'll DoEvaluate patients and develop personalized treatment plans.

Improve mobility, strength, balance, and overall function.

Educate patients and caregivers on exercises, safety, and disease management.

Monitor progress and adjust treatment plans to maximize outcomes.

Collaborate with physicians, nurses, and interdisciplinary team members.

Complete timely and accurate clinical documentation.

QualificationsGraduate of an accredited Physical Therapy program.

Current Physical Therapist license.

CPR certification.

Valid driver's license and auto insurance.

Home Health experience preferred.

HomeCare HomeBase experience preferred.
