Mercy is seeking a dedicated Physical Therapist to join our Healthcare & Medical Services team. In this role, you will assess, plan, and implement individualized therapy programs to restore function, improve mobility, and relieve pain for patients across diverse conditions. Responsibilities Perform comprehensive physical therapy evaluations, including musculoskeletal, neuromuscular, cardiopulmonary, and functional assessments. Develop and implement individualized, evidence-based treatment plans aligned with patient goals and physician recommendations. Provide therapeutic interventions such as therapeutic exercise, manual therapy, gait and balance training, neuromuscular re-education, and functional mobility training. Educate patients and families on diagnosis, prognosis, home exercise programs, safety, and strategies to maximize independence and prevent re-injury. Document evaluations, daily treatment notes, progress reports, and discharge summaries in the electronic medical record accurately and in a timely manner. Monitor and reassess patient progress, modifying treatment plans as needed to achieve optimal functional outcomes. Collaborate with physicians, nurses, occupational and speech therapists, and other team members to coordinate comprehensive, patient-centered care. Participate in quality improvement activities, safety initiatives, and adherence to all regulatory and professional practice standards. Maintain a clean, safe clinical environment and ensure proper use and maintenance of therapeutic equipment. Engage in ongoing professional development, including continuing education and potential pursuit of advanced clinical certifications.
