Fully support with QA reviewers / Clinical Director / Case Management . Qualifications Degree from an accredited Physical Therapy program Home health experience preferred Current and unrestricted VA Physical Therapy license Good organizational skills

Job Description:

Help patients with injuries and chronic health conditions regain their mobility, manage their pain and improve their quality of life. Company Car usage for traveling to visit patients Provide therapy services consistent with the patient plan of care Observe patient during treatment and report patient progress Act as a liaison and maintain contact with patients, family and Clinical Team to ensure effective care coordination Provide hands on care and education of the patient Documenting all visits via Software / Kinnser.net
