Job Description
Job Overview We are seeking a dedicated and compassionate Physical Therapist to join our healthcare team. The ideal candidate will provide exceptional patient care in Outpatient settings in our clinic, focusing on improving mobility, function, and quality of life for patients. This role involves developing personalized care plans, providing high level and compassionate one-on-one care, and collaborating with multidisciplinary teams to ensure optimal recovery outcomes. The position offers an opportunity to make a meaningful difference in the lives of patients while working within a supportive and professional environment. Responsibilities Assess patients' physical conditions through comprehensive evaluations. Develop and implement individualized care plans tailored to patients' needs, including discharge planning and therapy goals. Provide various therapy treatments to improve mobility and reduce pain. Monitor patient progress regularly and adjust treatment plans accordingly to ensure effective progress. Collaborate with physicians, nurses, caregivers, and other healthcare professionals to coordinate patient care. Educate patients and their families on exercises, safety measures, and health maintenance strategies to promote independence. Document patient interactions accurately using EMR systems while maintaining compliance with healthcare regulations. Participate in interdisciplinary team meetings to facilitate comprehensive patient discharge planning and continuity of care. Stay current with advancements in physical therapy techniques. Qualifications Valid licensure as a Doctor of Physical Therapist with experience in Pelvic Health or Geriatrics preferred. Experience working in acute care hospitals, inpatient units, outpatient clinics, or home & community care environments. Strong knowledge of physiology, anatomy, medical terminology, orthopedics. Familiarity with EMR systems for documentation and patient management. Proven ability to deliver patient-centered care across diverse settings. Excellent communication skills for effective collaboration with multidisciplinary teams and educating patients/families. Ability to adapt treatment approaches based on individual patient needs.
