What You'll Do Provide skilled physical therapy services and individualized therapeutic exercises to patients in their homes. Evaluate patients and develop personalized treatment plans based on their individual needs and goals. Monitor patient progress and adjust treatment plans as needed. Communicate patient progress and outcomes with physicians, case managers, families, and other members of the care team. Educate patients and caregivers on exercises, mobility, safety, and strategies to support continued progress at home. Promote patient safety, comfort, and independence through proper body mechanics and appropriate safety practices. Complete accurate and timely clinical documentation in accordance with applicable regulations and organizational standards. Maintain high professional and ethical standards consistent with the American Physical Therapy Association (APTA). What We're Looking For Graduate of an accredited Physical Therapy program. Current Physical Therapist license in the state of Utah. Strong communication and interpersonal skills. Professional, dependable, and organized. Commitment to patient privacy and confidentiality. Compassionate approach to patient care. Ability to work independently while also collaborating effectively with patients, families, and the care team.
