Day-to-day interactions will emphasize teamwork, communication, and shared goals to enhance patient outcomes. Key Responsibilities Conduct thorough evaluations of resident mobility, strength, and functional capabilities through standardized assessments. Develop, implement, and modify personalized treatment plans focused on pain management, mobility enhancement, and fall prevention based on individual needs. Administer diverse therapeutic interventions, including manual therapy techniques, therapeutic exercises, and balance training to facilitate recovery. Utilize assistive devices effectively and recommend appropriate mobility aids to empower residents' independence. Collaborate closely with interdisciplinary teams, including nursing staff and occupational therapists, to ensure seamless integration of care for each resident. Educate residents and their families on best practices for home exercise programs, mobility strategies, and safety precautions to optimize recovery at home. Maintain precise and timely documentation to ensure compliance with Skilled Nursing Facility regulations and best practices within the industry. Patient Population / Client Focus Your primary focus will be on elderly residents facing challenges due to acute injuries, chronic conditions, or post-surgical recovery. Our facility caters to a diverse group of patients, which may include those with orthopedic issues, neurological disorders, and general debilitation due to age-related factors. Understanding the unique needs of this population and applying a compassionate approach will be essential in enhancing their quality of life. Qualifications Must possess a valid and active Physical Therapy license in Virginia. A Doctor of Physical Therapy (DPT) degree or a Bachelor's/Master's degree in Physical Therapy from an accredited program is required. Preferred Experience At least two years of experience working in a Skilled Nursing Facility or geriatric rehabilitation setting is preferred. Knowledge and familiarity with long-term care regulations and rehabilitative care best practices pertinent to the elderly population. Compensation and Benefits The annual salary for this position ranges between $90,000 and $120,000, commensurate with experience and qualifications.
