Work Environment This role is based onsite within the community as part of our home care service, allowing physical therapists to deliver personalized, hands-on treatment in patients' homes. Key Responsibilities As an Orthopedic Physical Therapist, your responsibilities will include: Conducting thorough assessments of patients to determine their functional limitations, evaluating medical history and performing physical examinations. Developing individualized, evidence-based treatment plans aimed at improving mobility, function, and overall health. Collaborating with healthcare providers to ensure a seamless continuum of care for each patient, engaging in comprehensive discussions about treatment progress and modifications as necessary. Providing clear and empathetic communication to patients and their families regarding the treatment regimen, expected outcomes, and discharge planning. Maintaining meticulous documentation of patient evaluations, treatment plans, progress notes, and charges in compliance with regulatory guidelines. Supervising and guiding support staff, including Physical Therapy Assistants (PTAs) and aides, while promoting best practices in patient care. Participating in community outreach and marketing initiatives to enhance our service offerings and educate the community on the benefits of physical therapy. Attending department meetings and engaging in ongoing professional development to stay informed about the latest practices and innovations in physical therapy. Patient Population / Client Focus Your work will primarily focus on orthopedic patients, ranging in age and background. This role requires a deep understanding of the complexities of orthopedic conditions and the ability to tailor interventions specifically for each patient's unique needs. You will engage with diverse patient populations, fostering a therapeutic relationship that respects cultural differences and promotes inclusivity in care delivery. Qualifications To excel in this position, candidates must possess the following qualifications: The required education includes a Doctorate in Physical Therapy. A current state license in Physical Therapy is mandatory. Basic Life Support (BLS) certification is required to ensure patient safety during treatment. Excellent communication skills, both written and verbal, to facilitate effective interactions with patients and healthcare professionals. Proficiency in computer skills for documentation and reporting purposes. Preferred Experience While not mandatory, candidates with experience in home health care settings and familiarity with electronic health record systems are highly advantageous. Previous roles in orthopedics or rehabilitation settings are preferred, as is experience in patient education and community outreach.
