Home Health Join Our Team Star Light Home Health is seeking a compassionate, dependable, and motivated Physical Therapist Assistant (PTA) to provide skilled physical therapy services to patients in their homes under the direction and supervision of a licensed Physical Therapist. This position is a great opportunity for a PTA who enjoys one-on-one patient care, flexible scheduling, and helping patients improve their strength, mobility, safety, and independence in their own homes. Responsibilities The Physical Therapist Assistant will: Provide skilled physical therapy treatments according to the established Physical Therapist plan of care. Work under the direction and supervision of the supervising Physical Therapist. Provide therapeutic exercises, strengthening, balance training, gait training, transfer training, and other interventions within the PTA scope of practice. Assist patients with improving functional mobility, endurance, and safety. Reinforce proper use of walkers, canes, and other prescribed assistive devices. Reinforce individualized home exercise programs established by the Physical Therapist. Provide patient and caregiver education regarding fall prevention, home safety, mobility, and prescribed exercises. Observe and document the patient's response to treatment and progress toward established therapy goals. Promptly communicate significant changes in patient condition, functional status, pain, safety, or ability to participate in therapy to the supervising Physical Therapist and appropriate agency staff. Coordinate with the Physical Therapist and interdisciplinary care team to support continuity of care. Complete accurate and timely clinical documentation according to agency requirements. Maintain patient confidentiality and follow applicable HIPAA requirements. Follow Star Light Home Health policies, procedures, and applicable state and federal requirements. Qualifications Current Physical Therapist Assistant license in the state of Colorado. Graduate of an accredited Physical Therapist Assistant program. Current CPR/BLS certification. Valid driver's license and reliable transportation. Ability and willingness to travel to patients' homes. Strong communication and organizational skills. Ability to work independently while maintaining appropriate communication with the supervising Physical Therapist and agency team. Home health experience preferred, but not required.
