Job Summary We are seeking a dedicated and skilled Home Health Physical Therapist Assistant to join our healthcare team. Duties Assist licensed physical therapists in developing and executing individualized treatment plans tailored to each patient's needs. Implement physical therapy treatments including manual therapy and other modalities to improve mobility and reduce pain. Conduct clinical assessments and document patient progress accurately following clinical documentation standards. Operate medical equipment safely and effectively during therapy sessions, ensuring proper usage and maintenance. Collaborate with healthcare professionals, caregivers, and patients to coordinate care plans and facilitate discharge planning. Maintain comprehensive medical documentation within electronic health records (EHR) systems, ensuring compliance with legal and clinical standards. Communicate clearly with clients and caregivers to educate them on exercises, safety precautions, and post-treatment care instructions. Support patients across various populations including geriatrics, pediatrics, orthopedics, and those recovering from acute or chronic conditions. Experience Prior experience working in home health settings is highly preferred Demonstrated knowledge of physiology, anatomy, and medical terminology relevant to physical therapy practices. Hands-on experience with clinical assessment documentation and medical documentation standards. Skilled in the operation of physical therapy equipment used for manual therapy and therapeutic interventions. Experience delivering care for diverse patient populations such as geriatrics, pediatrics, orthopedics, or post-acute care environments. Proven ability to communicate effectively with patients of varying ages and backgrounds while maintaining professionalism and compassion.
