Job Description
Renewal Rehab seeks a compassionate Physical Therapist Assistant to support high-quality rehab services for seniors in skilled nursing and post-acute care settings. Working under the direction of a Physical Therapist, you will implement treatment plans, lead therapeutic exercises, track patient progress, and help patients maximize mobility, safety, and independence. You'll collaborate closely with an interdisciplinary team in a supportive, patient-centered culture that values integrity, respect, and open communication, with opportunities for mentorship, professional growth, and meaningful daily impact. Responsibilities Implement PT treatment plans under supervision of a licensed Physical Therapist Provide therapeutic exercises, gait and balance training, and functional mobility activities Monitor, document, and report patient progress and responses to treatment Assist with patient evaluations by gathering data and performing delegated tests Educate patients and families on home exercises, safety, and functional goals Collaborate with nursing, therapy, and medical staff to coordinate care Maintain accurate, timely, and compliant clinical documentation Ensure safe use of equipment and adherence to infection control protocols Participate in care conferences and discharge planning as appropriate Support a patient-centered, respectful, and compassionate care environment Required Skills Therapeutic exercise implementation Gait and balance training Functional mobility training Use of assistive devices and adaptive equipment Rehab documentation and EMR use Vital signs monitoring and patient observation Patient and family education Knowledge of rehab protocols for geriatric populations Infection control and safety procedures Interdisciplinary team collaboration
