Responsibilities Implement physical therapy treatment plans under the supervision of a licensed Physical Therapist Assist patients with therapeutic exercises, gait training, and functional mobility tasks Monitor, document, and report patient progress and response to treatment Collaborate with an interdisciplinary team to support individualized care plans Educate patients, families, and caregivers on exercises, safety, and home programs Ensure accurate, timely, and compliant clinical documentation Maintain a safe, clean, and supportive treatment environment Assist with discharge planning and recommendations for continued care or equipment needs Communicate effectively with nursing, medical staff, and families regarding patient status Uphold company standards for ethical practice, integrity, and quality outcomes in senior care Required Skills Therapeutic exercise implementation Gait training and balance interventions Functional mobility training (transfers, bed mobility) Geriatric rehabilitation techniques Therapeutic modalities (e.g., heat, cold, ultrasound as permitted) Clinical documentation in EMR systems Patient and family education Interdisciplinary care coordination Falls risk assessment and safety techniques Range of motion and strengthening programs
