Renewal Rehab seeks a compassionate Physical Therapist Assistant to support skilled nursing and post-acute care patients. The PTA will implement treatment plans under the direction of a Physical Therapist, focusing on improving mobility, strength, and functional independence for seniors. Responsibilities Implement physical therapy treatment plans under PT supervision Provide therapeutic exercises, balance and gait training to seniors Assist with functional mobility, transfer training, and fall-prevention interventions Observe, measure, and document patient progress and response to treatment Communicate patient status and recommendations to supervising PT and interdisciplinary team Educate patients, families, and caregivers on home exercises and safety Ensure accurate, timely, and compliant documentation in EMR or facility systems Maintain safe treatment environment and proper use of therapy equipment Collaborate with nursing and rehab team to support discharge planning and care goals Uphold ethical, patient-centered standards and facility policies Required Skills Therapeutic exercise implementation Gait and balance training Functional mobility and transfer training Geriatric rehabilitation techniques Skilled nursing/post-acute care practices Patient assessment and observation skills Clinical documentation and EMR use Patient and family education Interdisciplinary team collaboration Knowledge of safety and infection control standards
