Implement and monitor prescribed treatment plans, assist with therapeutic exercises, gait training, transfers, and functional activities. Prepare and set up equipment, apply modalities as directed, and ensure patient safety during treatments. Document patient progress, response to therapy, and communicate findings to the supervising PT and care team. Maintain infection control, device care, and appropriate documentation to support care coordination and discharge planning. Collaborate with nurses, therapists, and other staff to optimize patient outcomes and quality of life. Requirements Active Physical Therapy Assistant license in the state of New Hampshire or eligible for licensure. Associate degree in Physical Therapy Assisting (or equivalent) commonly required for PTA practice. Prior experience in LTAC or skilled nursing settings is preferred but not required. Ability to work in a long-term care environment and meet guaranteed hours for the assignment. Strong communication and documentation skills; ability to collaborate with a multidisciplinary team.
