Job Description

Tender Touch Rehab Services is seeking a compassionate Physical Therapist Assistant to provide high-quality, evidence-based rehab to seniors in skilled nursing and long-term care settings. Working under the direction of a Physical Therapist, you will carry out individualized treatment plans, document progress, and collaborate closely with nursing, rehab, and medical teams to improve function, independence, and quality of life. Responsibilities Implement physical therapy treatment plans under PT supervision Provide therapeutic exercises, gait training, and functional mobility activities Assist with balance, strength, and endurance training for seniors Measure and document patient progress and response to treatment Educate patients, families, and caregivers on home exercise and safety Collaborate with PTs, OTs, nursing, and medical staff on care plans Ensure safe use of equipment and adherence to infection control standards Participate in care conferences and provide clinical input Contribute to discharge planning and recommendations for continued care Uphold patient dignity, compassion, and respect in all interactions Required Skills Therapeutic exercise implementation Gait and balance training Functional mobility training Geriatric rehabilitation techniques Skilled nursing/long-term care rehab Safe patient handling and transfers Rehab documentation and EMR use Patient and family education Vital signs monitoring during treatment Infection control and safety procedures
