Job Description

Tender Touch Rehab Services is seeking a dedicated Physical Therapist Assistant to join our on-site rehab team serving skilled nursing and long-term care residents. In this role, you will deliver hands-on treatments under the direction of a Physical Therapist, focusing on improving mobility, function, and safety for geriatric and sub-acute patients. You'll implement individualized treatment plans, document progress, and collaborate closely with nursing and therapy staff. Responsibilities Provide physical therapy treatments under the supervision of a licensed Physical Therapist for geriatric and sub-acute residents Carry out individualized treatment plans including therapeutic exercise, gait training, transfers, and balance activities Accurately document treatments, patient responses, and progress in the EMR system within required timeframes Assist with patient assessments by reporting functional status, changes, and safety concerns to the supervising PTEducate patients, families, and caregivers on exercises, mobility strategies, and safety techniques Collaborate with interdisciplinary team members, including nursing and occupational and speech therapists, to support coordinated care Maintain compliance with regulatory, facility, and company policies, including infection control and documentation standards Participate in care conferences, in-services, and ongoing clinical education to support professional growth Help maintain therapy equipment and treatment areas in a safe, clean, and organized condition Contribute to a compassionate, patient-centered environment that promotes dignity, independence, and quality of life for residents Required Skills Therapeutic exercise implementation Gait and balance training Transfer and mobility assistance Therapeutic modalities application Care plan documentation (EMR) Functional outcome measurement Patient and family education Interdisciplinary care coordination Infection control practices Fall prevention techniques
