Job Description Metro is looking for a motivated and passionate per diem Physical Therapist Assistant to provide in-home care under the supervision and direction of a licensed Physical Therapist. •see patients any 7 days of the week Preference given to see patients in your primary geographic area

Responsibilities:

Accept and treat patients within your designated geographic area Provide physical therapy treatments to patients in their homes, as outlined in the licensed PT's plan of care Monitor and report changes in patient status to the supervising PT in a timely manner Communicate effectively with patients, caregivers, and interdisciplinary team members to ensure coordinated, effective care Instruct patients, family members, caregivers, and Home Health Aides in exercises, safety awareness, and use of assistive devices as directed by the supervising PT Assist the PT in updating care plans based on patient progress and response to treatment Document patient treatments and observations accurately and promptly, in accordance with agency and regulatory standards Participate in case conferences and contribute to the care planning and discharge process, in collaboration with the PT Adhere to all federal, state, and local regulations as well as agency policies and professional standards of practice Qualifications Physical Therapist Assistant Program Graduate Licensed Physical Therapist Assistant in the state of New York or eligible.

Mandatory Certifications:

First Aid, AED, & CPR Certifications. Home health experience is preferred but not required

•We will provide you with a mentor to help with documentation and compliance. Ability to travel to patients' homes within the assigned geographic territory
