Job Description
Overview We are seeking a dedicated and skilled Physical Therapist (PT) to join our healthcare team. The ideal candidate will possess a comprehensive understanding of physiotherapy practices, with a focus on patient-centered care and rehabilitation. In this role, you will evaluate patients, develop individualized treatment plans, and implement therapeutic interventions to promote recovery and improve quality of life. Your expertise will contribute to a multidisciplinary approach, collaborating with healthcare professionals across various settings to deliver optimal patient outcomes. Responsibilities Conduct thorough assessments of patients' physical conditions, including mobility, strength, and functional capabilities. Develop and implement personalized treatment plans that align with clinical guidelines and patient needs. Provide hands-on therapy techniques such as trigger point therapy, manual therapy, and other modalities to facilitate recovery. Monitor patient progress regularly, adjusting care plans as necessary to ensure effective rehabilitation outcomes. Document all treatment activities accurately in electronic health records (EHR) systems following clinical documentation standards. Collaborate closely with physicians, nurses, and other healthcare providers to coordinate care and discharge planning. Educate patients and their families on exercises, activity modifications, and home health strategies to support ongoing recovery. Ensure compliance with healthcare regulations and maintain high standards of patient safety throughout all procedures. Skills Strong knowledge of physiology, anatomy, and medical terminology relevant to physiotherapy. Experience working with diverse patient populations including geriatrics, pediatrics, neurology, orthopedics, and inpatient or outpatient settings. Proficiency in outpatient clinic experience, hospital experience, home health care, and outpatient rehabilitation centers. Skilled in developing treatment plans for patient rehabilitation across various environments such as inpatient units or outpatient clinics. Familiarity with clinical documentation standards and electronic health records (EHR) or electronic medical records (EMR) systems for accurate record keeping. Ability to perform discharge planning effectively while collaborating with healthcare teams for seamless patient transitions. Knowledge of physiotherapy techniques including trigger point therapy and physiotherapy modalities tailored for specific conditions like orthopedics or neurology. Excellent client communication skills to motivate patients and explain treatment procedures clearly. Working knowledge of home & community care practices to support patients outside clinical settings.
