San Antonio, TX TheraCare is seeking a licensed Physical Therapist Assistant (PTA) for a full-time home health opportunity providing in-home physical therapy services to adult and geriatric patients throughout San Antonio and surrounding areas. This full-time home health PTA position is ideal for clinicians looking for flexibility, autonomy, and the ability to provide one-on-one patient care in the home setting. Therapists enjoy creating their own schedules while helping patients improve mobility, strength, safety, and overall independence in the comfort of their homes. Job Responsibilities Provide skilled home health physical therapy interventions under the supervision of a licensed Physical Therapist (PT) Implement and carry out individualized Plans of Care (POC) developed and delegated by the supervising PT Treat patients with orthopedic, neurological, cardiac, and post-surgical conditions Administer therapeutic exercises, gait training, balance training, transfer training, and functional mobility activities Apply manual therapy techniques and non-complex modalities including heat, cold, electrical stimulation, and other physician-ordered treatments Educate patients and caregivers on home exercise programs (HEP), fall prevention, and safe mobility strategies Monitor patient response to treatment and promptly report changes in condition to the supervising Physical Therapist Complete accurate and timely electronic documentation in compliance with Medicare home health regulations Follow infection control standards and home health safety protocols Participate in QAPI and agency performance improvement initiatives This Full-Time home health PTA job in San Antonio, TX, is ideal for clinicians seeking flexible physical therapist assistant jobs with in-home patient care.What We Offer:
