LCME, AOA
Commission on Osteopathic College Accreditation, or an ACGME-recognized accrediting body. Foreign-trained physicians must demonstrate equivalency via permanent ECFMG certification, a Fifth Pathway certificate, or successful completion of the USMLE. Training & Experience Successful completion of an ACGME- or AOA-approved internship and residency program in Family Medicine, plus a minimum of 1 year of supervised clinical experience immediately following the
M.D./D.O.
degree. Minimum 3 years of experience in a family practice setting within the preceding 4 years, OR completion of a Family Practice residency within the preceding 2 years. Board Certification & Licensure Board Certification in Family Practice. Current, full, active, and unrestricted license to practice medicine in the United States. Certifications & Registration Current BLS certification. Valid, current DEA registration prior to performing contract services. References Two letters of recommendation, written within the preceding 2 years, from practicing providers, supervisors, or program administrators. Work Authorization Must be a U.S. Citizen.
