Job Description

A physician group is seeking a quality-focused physician to join its post-acute care team serving skilled nursing facilities throughout the Houston area, including Kingwood, Humble, and Spring. This is a full-time, five-day-per-week opportunity providing care in skilled nursing facilities. The position offers strong clinical support, comprehensive training, autonomy, and attractive earning potential. Compensation is fee-for-service with uncapped productivity-based income potential. A separate post-acute physician opportunity is also available in Baytown, Texas. In these role, physicians provide high-quality care to diverse patient populations while collaborating with experienced multidisciplinary teams. The positions also offer the opportunity to build meaningful relationships with patients and their families while helping patients through important transitions in care. Support and Development Physicians are supported by an experienced clinical and operational leadership team, including a comprehensive three-month onboarding program, a dedicated clinical field mentor, and support from clinical experts in post-acute medicine. Growth and leadership opportunities are available, along with additional training focused on quality improvement and collaboration with facility leadership. Key Responsibilities Perform comprehensive patient assessments, including medical histories and physical exams Develop, implement, and adjust individualized care plans based on patient needs Monitor patient progress and coordinate care to optimize outcomes Collaborate closely with physicians, nursing staff, and other healthcare professionals Educate patients and families on health management and preventive strategies Maintain accurate and timely medical records Why consider this opportunity? Pay:
$190,000.00 - $225,000.00 per year
