Covington, Conecuh, and Butler Counties, Alabama Are you a compassionate and experienced Registered Nurse looking to make a meaningful impact in home health care? We are seeking a dedicated Home Health RN to provide high-quality patient care throughout Covington, Conecuh, and Butler counties. This role offers the opportunity to work independently while helping patients achieve the best possible health outcomes in the comfort of their homes.

Yes, between patient visits Key Responsibilities Provide skilled nursing care to patients in their homes in accordance with physician orders and agency protocols. Conduct comprehensive patient assessments and develop individualized care plans. Monitor patient conditions and document all care provided accurately and timely. Educate patients and caregivers regarding disease management, medications, treatments, and overall health maintenance. Coordinate care with physicians, therapists, social workers, and other members of the healthcare team. Perform wound care, medication management, and other skilled nursing interventions as required. Maintain compliance with state, federal, and agency regulations. Complete all required documentation using Homecare Homebase (HCHB). Required Qualifications Active Registered Nurse (RN) license in the State of Alabama. Current BLS/CPR certification. Previous Home Health nursing experience. Experience with Homecare Homebase (HCHB) documentation. Valid driver's license, reliable transportation, and current vehicle insurance. Strong clinical assessment and critical thinking skills. Ability to work independently and manage a patient caseload efficiently. Preferred Qualifications Candidates with the following skills will stand out: Advanced wound care experience. Wound VAC management. IV site care and maintenance. Venipuncture skills. Excellent communication and patient education abilities. Strong organizational and time-management skills.
