Overview We are seeking a dedicated and highly skilled Registered Nurse (RN) to join our team in a full-time capacity, working (NOC) shift at our skilled nursing facility. The ideal candidate will possess a comprehensive understanding of patient care within long-term care, acute care, and specialized medical environments. This role offers an opportunity to provide compassionate, high-quality nursing services while utilizing advanced clinical skills across diverse medical disciplines. The RN will be instrumental in ensuring optimal patient outcomes through meticulous assessment, planning, and execution of individualized care plans. Responsibilities Perform thorough patient assessments, including vital signs, physical examinations, and detailed medical histories to inform care strategies. Administer medications and injections accurately, adhering to prescribed protocols and maintaining strict aseptic techniques. Manage complex medical procedures such as IV infusions, catheterizations, wound suturing, and ventilator management with precision and safety. Coordinate with interdisciplinary teams to develop and implement comprehensive care plans tailored to each patient's needs, including memory care, hospice & palliative medicine, and behavioral health considerations. Document all patient interactions and treatments meticulously within Electronic Health Records (EHR) systems such as Epic or Cerner, ensuring compliance with HIPAA regulations. Monitor patients' responses to treatments, adjusting interventions as necessary to optimize health outcomes in settings like ICU, trauma centers, or long-term care facilities. Provide education to patients and families regarding medication administration, disease management, and post-discharge care planning. Participate in quality improvement initiatives aligned with NCQA standards and infection control protocols to uphold the highest standards of patient safety and care excellence. Experience Proven experience working in diverse clinical settings including hospital medicine, emergency medicine, critical care (ICU), trauma centers (Level I & II), or neonatal/pediatric intensive units (PICU). Demonstrated expertise with EMR/EHR systems such as Epic or Cerner for accurate documentation and data management. Extensive background in managing complex cases involving ICD coding (ICD-9/10), CPT coding, laboratory procedures, and specimen processing. Prior experience providing specialized nursing services such as dialysis, ostomy care, tube feeding management, or infectious disease care is highly desirable. Familiarity with advanced nursing procedures including airway management, ventilator support, sterile processing techniques, and aseptic techniques. Experience working with individuals with disabilities or developmental delays in settings like assisted living or memory care units is advantageous. Knowledge of Workers' Compensation law related to occupational health cases combined with experience in discharge planning and case management enhances patient advocacy efforts.
