A Registered Nurse provides medical care and treatment, educates patients about their conditions, and provides emotional support to patients and families. Works under the supervision of a physician or other specialist, and serves as the primary point of care for patients in a hospital setting. May specialize in emergency room work, or treatment for a particular condition, or specialize in working with infants, the elderly or other patient groups. May work as an advanced practice specialists and prescribe medications in addition to offering specialty care.

Job Description
Minimum Data Set (MDS) Monday-Friday/ 8:30A-5PM/
•PDPM or MDS Experience required. We are looking for an experienced, organized and professional MDS Nurse for a skilled nursing facility to join our growing organization in Glendora, CA.
Duties:
Coordinates the interdisciplinary team in timely completion of the assessments. Utilizes MDS Scheduler to plan and set MDS Assessment Reference Dates (ARDs). Communicates ARD selection for specific MDSs to IDT. Accomplishes the MDS sections as assigned/fulfill CAAs as required. Completes Care Plan items and attends Care Plan meetings as assigned. Finalizes diagnosis coding and creates diagnosis sheets. Monitors the overall documentation in the medical record to validate that it supports MDS coding. Participates in skilled utilization and State Case Mix management as assigned. Delivers teaching and training for MDS item completion to interdisciplinary team members that have responsibility for MDS item completion. If Registered Nurse, signs off on Minimum Data Set (MDS) for each resident within the facility.
Requirements:
Must possess a current, unencumbered, active license to practice as an RN or LVN in California. Experience with PDPM processes and at least two years' experience working in a long-term care setting. Current CPR certification. Must pass pre-employment background check and physical.
