We service multiple cities throughout Los Angeles County Description Responsible for planning, organizing, and directing home care services and is experienced in nursing, with an emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities. Responsibilities and Duties Patient Care Completes an initial assessment of patient and family to determine home care needs. Regularly re-evaluates patient nursing needs. Initiates the plan of care and makes necessary revisions as patient status and needs change. Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process. Administers medications and treatments as prescribed by the physician. Provides health care instructions to the patient as appropriate per assessment and plan of care. Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.

Qualifications and Skills Basic Qualifications:

Current and unrestricted RN License in the State of California. 1 yr experience in home health or acute setting Current BLS and ACLS certification. Clinical experience in an inpatient or acute setting. Problem solving skills; the ability to systematically analyze problems, draw relevant conclusions and devise appropriate courses of action. Excellent verbal and written communication skills; ability to speak clearly and concisely, conveying complex or technical information in a manner that others can understand, as well as ability to understand and interpret complex information from others. Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order. Must be able to pass a criminal background check and drug test.

Preferred Qualifications:

1 year of home health experience. Bachelor's degree in nursing. Experience in acute long-term care, acute rehabilitation, or skilled nursing facilities. Benefits Flexible schedule
