Key Responsibilities:

Perform annual and post-discharge home health risk assessments for Medicare Advantage plan members residing in various locations, including Grand Junction, Fruita, and Palisade Review prior and current medical history, evaluate ongoing health conditions, and assess medication history Educate patients about their medical conditions and medication usage Identify gaps in care and provide actionable recommendations to address them Focus exclusively on assessments; no treatment planning or prescribing medications required

Qualifications:

Active Nurse Practitioner (NP) License in Colorado Board Certification (AANP or ANCC) required Master of Science in Nursing (MSN) from an accredited program Current CPR certification (BLS) Minimum 6 months of recent Nurse Practitioner experience preferred Enjoy the autonomy of per-evaluation compensation ($80-$90 depending on assessment), plus additional mileage stipend and payment for administrative time.
