Job Description

Excelcare Newark is seeking an MDS Coordinator who is responsible for overseeing and coordinating the completion of accurate and timely Minimum Data Set (MDS) assessments in accordance with state and federal regulations. This role ensures regulatory compliance, supports quality care outcomes, and collaborates with interdisciplinary teams to optimize resident care planning, reimbursement, and quality measures. MDS Coordinator Responsibilities:
Coordinate MDS assessments and ensure timely, accurate completion of all required documentation. Develop and update care plans in collaboration with the interdisciplinary team. Maintain compliant patient records aligned with regulatory standards. Analyze MDS data to support quality improvement efforts. Educate and support staff on MDS processes and care planning best practices.
MDS Coordinator Qualifications:
Active RN license in the state. Minimum 2 years of MDS experience in a skilled nursing setting. Strong attention to detail with excellent documentation and analytical skills. Effective communication skills for working with residents, families, and interdisciplinary teams. Ability to work independently in a fast paced environment and manage multiple priorities.
