Deliver quality patient care in a dynamic, community-based setting by performing home health risk assessments for Medicare Advantage plan members within Cassia County. This per diem position empowers a Nurse Practitioner to create their own schedule, allowing for flexibility and work-life balance. Assessments are conducted in members homes across Burley, Oakley, Declo, and other locations in the county, with each visit taking approximately 45 minutes and focusing on either annual health evaluations or post-discharge follow-ups.

Key Responsibilities:
Complete annual health risk assessments tailored to Medicare Advantage plan guidelines Thoroughly evaluate patients by reviewing prior/current medical history, medical conditions, and medication usage Offer clear education to members on their health conditions and medication regimens Identify care gaps and recommend appropriate steps to resolve them Role does not require creating treatment plans or prescribing medications
Qualifications:
Active Nurse Practitioner license in Idaho Current CPR/BLS certification MSN degree from an accredited school of nursing Board Certification through AANP or ANCC Minimum of 6 months of recent NP experience preferred This opportunity offers earnings of $80-$90 per evaluation, which includes a mileage stipend and payment for administrative time.
