Responsibilities:
Conduct annual home health risk assessments in patients homes Evaluate clinical data including medical history, current medical conditions, and medication history Educate patients on their conditions and prescribed medications Identify care gaps and suggest evidence-based recommendations to improve patient outcomes No responsibilities for treatment plan creation or medication prescribing
Qualifications:
Active Nurse Practitioner license in Idaho Current BLS (CPR) certification Board Certified by AANP or
ANCC MSN
degree from an accredited nursing program At least six months of recent NP experience preferred This position offers compensation of $80$90 for each completed evaluation, as well as a mileage stipend and additional compensation for eligible administrative time.
