No call Requirements Active Illinois RN license Home Health experience required OASIS/Start of Care experience strongly preferred Active BLS or willingness to obtain Valid driver's license, reliable vehicle, and auto insurance COVID-19 and flu vaccination required Strong Med Surg experience with some Home Health experience may be considered Responsibilities Provide skilled nursing care in patients' homes, complete assessments and OASIS documentation, develop Plans of Care, educate patients/caregivers, coordinate with the care team, and perform some telehealth/virtual visits. Mileage reimbursement provided. CA02
