HOME HEALTH REGISTERED NURSE

(RN)

Responsibilities:

Conduct initial and ongoing patient assessments. Develop and update individualized plans of care. Complete OASIS assessments and Start of Care documentation. Provide patient and caregiver education. Coordinate care with physicians, CNAs, and other healthcare professionals. Maintain accurate documentation and follow nursing standards and safety regulations.

Requirements:

Active Illinois RN license. Home health experience required; strong Med-Surg experience with some home health experience may be considered. Active BLS or willingness to obtain. Valid driver's license and car insurance. COVID-19 and flu vaccinations required; declinations not accepted. OASIS and Start of Care experience strongly preferred. Ability to travel within the assigned territory.
