Key Responsibilities:

Conduct home health risk assessmentseach visit averaging about 45 minutesvarying from annual health reviews to post-discharge follow-ups Review and document detailed medical histories, current health status, and medication use Educate patients on their conditions and prescribed medications, supporting long-term wellness Detect any gaps in care and make actionable recommendations to help close them Focus solely on assessment and education; treatment planning and medication prescribing are not required

Qualifications:

Active Nurse Practitioner license in Indiana (IN) Current BLS certification (CPR) Board certified by AANP or ANCC Minimum of 6 months NP experience preferred Master of Science in Nursing (MSN) from an accredited institution Enjoy competitive compensation of $80$90 per evaluation, supplemented with a mileage stipend and additional pay for administrative time.
