Key Responsibilities:
Perform thorough home health risk assessments for Medicare Advantage plan members Assess patients current and past medical history, medical conditions, and medication usage Provide patient education about ongoing medical conditions and medications Identify potential gaps in care and offer recommendations to address these gaps No requirement to develop treatment plans or prescribe medications
Qualifications:
Active Nurse Practitioner license in Indiana required Current BLS (CPR) certification Board certified through
AANP or ANCC Preferred:
