A rewarding opportunity is available for skilled Nurse Practitioners to conduct home health risk assessments on a per diem basis. In this flexible role, you will visit Medicare Advantage plan members in Marion, Gas City, Upland, and other areas within Grant County, IN. Each assessment appointment averages 45 minutes and may encompass annual health evaluations or post-discharge visits.

Key Responsibilities:

Complete comprehensive home health risk assessments for Medicare Advantage plan members Evaluate patients prior and current medical history, health conditions, and medication usage Educate patients about their conditions and prescribed medications during in-home visits Identify gaps in care and offer recommendations to improve patient health outcomes No responsibility for developing treatment plans or prescribing medications

Qualifications:

Active Nurse Practitioner license in Indiana (IN) MSN degree from an accredited nursing school Board Certification through AANP or ANCC Current CPR (BLS) certification Minimum of 6 months of recent NP experience preferred You will receive competitive pay of $80-$90 per evaluation, with an additional mileage stipend and compensation for administrative time.
