Job Description

LifeBridge Health, Inc. seeks a Nurse Coordinator to lead and coordinate patient care services across its Baltimore-based health system. In this patient-centered role, you will manage care plans, oversee clinical workflows, and serve as a liaison among physicians, nurses, patients, and families. Responsibilities include care coordination, staff guidance, patient education, quality and safety monitoring, and documentation. You will collaborate with interdisciplinary teams to optimize outcomes, support community-focused initiatives, and contribute to process improvements in a mission-driven, innovative environment. Responsibilities Coordinate and manage patient care plans across interdisciplinary teams Serve as primary liaison among physicians, nurses, patients, and families Monitor patient progress, outcomes, and quality/safety indicators Support and guide nursing staff in daily clinical operations Provide patient and family education on care plans and discharge instructions Ensure accurate, timely clinical documentation and care transitions Participate in quality improvement and clinical practice initiatives Collaborate on community-focused and population health programs Required Skills Care coordination Clinical assessment Electronic Health Records (EHR) documentation Patient and family education Interdisciplinary team collaboration Quality and safety monitoring Discharge planning and transitions of care Time management and prioritization Data tracking and reporting Regulatory and compliance knowledge
