This role offers flexibility and meaningful work supporting patients and families during end-of-life care. Hiring for day shifts Monday
• Friday; 8:00 am-5:00 pm.

Key Responsibilities Complete hospice admissions, assessments, and reassessments Manage symptoms and implement individualized plans of care Educate and support patients, families, and caregivers Coordinate care with physicians and interdisciplinary teams Complete documentation, discharges, and pronouncements as needed Qualifications Active Michigan RN license 1+ year RN experience preferred Hospice or home health experience preferred Strong communication skills and compassionate bedside manner Reliable transportation; ability to pass background and drug screen Position Details Outpatient assignments Home
