Key Responsibilities:

Conduct comprehensive patient assessments and develop individualized care plans. Provide skilled nursing treatments and procedures as required. Maintain accurate and up-to-date patient documentation and records. Coordinate care and communicate effectively with patients, families, and interdisciplinary teams. Ensure compliance with all safety, infection control, and regulatory standards. Participate in patient education and advocate for patient needs.
