This role primarily involves coordinating and managing care for residents in an assisted living setting. The ideal candidate will be responsible for providing clinical oversight, ensuring residents receive appropriate care, and collaborating with healthcare teams to promote optimal health outcomes. This position offers an opportunity to make a meaningful difference in residents' lives while gaining valuable experience in case management within a supportive environment. Responsibilities Assess residents' health status and develop individualized care plans in collaboration with healthcare providers. Monitor and evaluate residents' progress, adjusting care plans as needed to meet changing health needs. Coordinate medical services, including scheduling appointments, medication management, and follow-up care. Serve as a liaison between residents, families, and healthcare teams to communicate updates and address concerns. Ensure compliance with all relevant healthcare regulations and facility policies. Document all care activities accurately and maintain detailed records of resident interactions and interventions. Provide education to residents and their families regarding health conditions, treatment options, and wellness strategies. Assist with daily living activities as needed, maintaining a compassionate and respectful approach at all times. Support the facility's safety protocols and contribute to creating a safe environment for residents and staff. Requirements Valid Registered Nurse (RN) license in the state of practice. Prior experience in assisted living or long-term care settings is preferred. Strong knowledge of healthcare regulations, medication management, and resident care standards. Excellent communication skills with the ability to collaborate effectively with multidisciplinary teams, residents, and families. Compassionate nature with a focus on resident-centered care. Ability to adapt quickly in a dynamic environment and handle multiple priorities efficiently. Basic computer skills for documentation and communication purposes. Availability for a temporary assignment lasting a few months, with flexibility to meet the needs of the facility. Pay:
$60.00 per hour Expected hours: No less than 40.0 per week
