This role primarily involves coordinating and managing care for residents in an assisted living setting. Responsibilities Assess residents' health status and develop individualized care plans in collaboration with healthcare providers. Serve as a liaison between residents, families, and healthcare teams to communicate updates and address concerns. Ensure compliance with all relevant healthcare regulations and facility policies. Prior experience in assisted living or long-term care settings is preferred. Strong knowledge of healthcare regulations, medication management, and resident care standards. Excellent communication skills with the ability to collaborate effectively with multidisciplinary teams, residents, and families. Compassionate nature with a focus on resident-centered care. Ability to adapt quickly in a dynamic environment and handle multiple priorities efficiently. Basic computer skills for documentation and communication purposes. Availability for a temporary assignment lasting a few months, with flexibility to meet the needs of the facility.
