Be a Nurse, Not a Caseload Manager As a Visit Nurse, your focus is delivering the skilled nursing care patients need in their homes. You'll work alongside RN Case Managers and an interdisciplinary clinical team while providing visits based on each patient's plan of care. That means your day can include assessments, wound care, medication management, disease education, post-hospitalization support, chronic disease management, and recognizing changes in condition. You still have documentation. You still have productivity expectations. You still have responsibility. The next is recovering after a hospitalization. Another needs medication education after several prescriptions changed. Later, you're assessing a patient with CHF, diabetes, COPD, or another chronic condition and determining whether what you're seeing requires additional intervention. The setting changes. What You'll Do Provide skilled nursing visits throughout your assigned territory Complete comprehensive nursing assessments Perform wound care and other ordered treatments Provide medication management and education Support patients following hospitalization, surgery, illness, or changes in condition Monitor chronic and medically complex conditions Recognize changes in condition and respond appropriately Educate patients, caregivers, and families Support admissions and additional skilled visits as needed Complete accurate and timely clinical documentation Communicate findings and concerns to RN Case Managers and physicians Collaborate with PTs, OTs, SLPs, social workers, and other care team members Participate in an on-call and weekend rotation You Already Have the Skills. Take Them Into the Field. We're interested in RNs from Home Health, Med/Surg, Acute Care, TCU, Skilled Nursing, Rehabilitation, Hospice, Post-Acute Care, and other hands-on clinical environments . If you know how to assess a patient, recognize when something isn't right, provide skilled treatments, educate families, and communicate effectively with a care team, you already have many of the skills that make a strong Visit Nurse. We'll help you learn the home health environment and give you an experienced clinical team to turn to when you need support.
What You Bring Required:
Active Minnesota Registered Nurse (RN) license Strong clinical assessment and critical-thinking skills Excellent patient and caregiver communication Confidence providing hands-on skilled nursing care Ability to work independently in patient homes Strong organization and time-management skills Valid driver's license and reliable transportation
