A unique opportunity is available for Nurse Practitioners skilled in home health assessments to work on a per diem basis. This flexible role involves conducting risk assessments for Medicare Advantage plan members throughout Jefferson County, MOincluding areas such as Arnold, Imperial, Festus, and Barnhart.

Flexible shifts available between Monday and Sunday Design your own work schedule Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

Perform comprehensive annual health risk assessments in patients homes Assess prior and current medical history, review medical conditions, and evaluate medication histories Educate patients on health conditions and medication management Identify gaps in care, providing recommendations to improve patient health outcomes Visits typically last approximately 45 minutes and may include routine annual evaluations or post-discharge assessments No treatment plan development or medication prescribing is required

Qualifications:

Active Nurse Practitioner License in Missouri Current CPR (BLS) certification Board Certification through either AANP or

ANCC MSN

from an accredited school of nursing Minimum of 6 months recent NP experience preferred In this role, you will be compensated per completed evaluationearning between $80-$90 per home health assessment, in addition to a mileage stipend and pay for administrative time.
