Job Description

Advance your career as a Nurse Practitioner specializing in home health services, delivering compassionate care through risk assessments for Medicare Advantage plan members in Dent County. In this per diem position, you will have the opportunity to provide essential health evaluations throughout the region, including areas such as Salem and Bunker. With the flexibility to tailor your schedule and a focus on clinical assessment rather than ongoing treatment, you can achieve meaningful impact with each patient interaction.
Schedule:
Flexible shifts available between Monday and Sunday Create your own work schedule Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
Conduct comprehensive annual home health risk assessments for Medicare Advantage plan members during home visits Review and evaluate prior/current medical and medication histories Educate patients regarding their medical conditions and medications to promote understanding and compliance Identify potential gaps in care, offering thoughtful recommendations to enhance patient outcomes Visits may range from annual assessments to post-discharge check-ups No need to create treatment plans or prescribe medications
Qualifications:
Valid Nurse Practitioner license in Missouri CPR certification (BLS) required Board Certification through AANP or ANCC Minimum of 6 months current NP experience preferred MSN degree from an accredited nursing program Your dedication will be rewarded with a competitive rate of $80$90 per evaluation, in addition to a mileage stipend and compensation for administrative tasks.
