Deliver high-quality patient care in a flexible, per diem role focused on home health risk assessments for Medicare Advantage plan members. Practitioners in this position travel to patient homes across Dent County, including communities like Salem and Bunker, to support continuity of care and improve patient outcomes. Each assessment takes approximately 45 minutes and may range from annual health evaluations to post-discharge follow-up visits.

Key Responsibilities:

Perform comprehensive home health risk assessments for Medicare Advantage members Review and document previous and current health history, conditions, and medications Educate patients about their diagnoses and prescribed therapies Identify gaps in care and recommend actionable steps No treatment planning or prescription duties required

Qualifications:

Current Nurse Practitioner license in Missouri Board certification via AANP or ANCC Current Basic Life Support (BLS/CPR) certification 6 or more months of recent Nurse Practitioner experience strongly preferred Master of Science in Nursing (MSN) from an accredited institution Enjoy competitive compensation of $80$90 per evaluation, with additional mileage stipend and paid administrative time.
