Key Responsibilities:

Perform annual home health risk assessments for Medicare Advantage health plans Thoroughly review and evaluate patients medical history, current conditions, and medication regimens Educate patients on medical conditions, medication management, and healthy practices Identify gaps in care and provide actionable recommendations to bridge those gaps

Qualifications:
